Position Overview:

We are seeking a compassionate and experienced Nurse Navigator to join our oncology team. The Nurse Navigator will play a crucial role in supporting patients as they navigate the complexities of their cancer diagnosis and treatment options. Working closely with physicians and other healthcare professionals, the Nurse Navigator will serve as a trusted advocate and resource for patients, providing education, coordination, and emotional support throughout their treatment journey.

 

Responsibilities:

 
Serve as the primary point of contact for patients diagnosed with cancer, offering personalized support and guidance from diagnosis through treatment and survivorship.
Provide comprehensive education to patients and their families regarding their diagnosis, treatment options, side effects, and supportive care services.
Coordinate appointments, tests, and treatments, ensuring seamless communication between healthcare providers and timely access to care.
Conduct thorough assessments of patients’ physical, emotional, and social needs, and develop individualized care plans to address their unique concerns.
Advocate for patients’ rights and preferences, ensuring that their voices are heard and respected throughout the healthcare process.
Collaborate with multidisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to facilitate coordinated and holistic care.
Monitor patients’ progress and outcomes, providing ongoing support, education, and symptom management as needed.
Participate in community outreach and education initiatives to raise awareness about cancer prevention, screening, and early detection.

 

Qualifications:

Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
Current Registered Nurse (RN) license in any US state.
Minimum of 10 years of clinical experience in oncology nursing, with a strong understanding of cancer diagnosis, treatment modalities, and supportive care interventions.
Certification in Oncology Nursing (OCN) preferred.
Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with patients and families.
Strong organizational and problem-solving abilities, with a proactive approach to patient care coordination.
Compassionate and empathetic demeanor, with a commitment to providing patient-centered care.
Ability to work collaboratively in a fast-paced, multidisciplinary team environment.
Proficiency in electronic health record (EHR) systems and Microsoft Office suite.
